Please use this form to indicate which of our Springtime 2025 workshops your child would like to attend.

Please read carefully and complete a form for each child attending. You are welcome to bring printed completed forms on the day or email them back to us no later than 8pm the evening before the workshop. If you are unable to print and cannot send a completed form via email, please let us know and we will have some printed copies available when you drop your child off. It would be great to have a note of our child/children's name/s before arrival so that we can have name tags ready so please drop us a line if you are planning to bring your printed consent form along with you on the day.
